What to Expect
Patients referred for a memory evaluation first view the required Memory Orientation Class online, during which normal age-related changes and causes of memory problems are discussed. Family members are asked to complete a questionnaire about their loved ones’ memory loss symptoms prior to the appointment.
An in-person evaluation is scheduled following the required Memory Orientation class. During this appointment, patients and caregivers will meet with the Memory Center team, which consists of physicians, nurse practitioners, social workers, and pharmacists. The visit will include a detailed history, memory assessment and physical examination, medication review, and discussion about any condition that is identified. If further tests are necessary, they will be scheduled and you will receive specific instructions.
The Memory Center team will then provide follow-up resources and evaluations tailored to your specific needs. Our team members will work closely with your primary care provider to manage your health conditions.
