Life Care Planning – First Steps

Life care planning begins with choosing someone to speak on your behalf if you were ever unable to communicate for yourself.  This class can assist you with exploring your values and goals for care.  You will also have an opportunity to complete an Advance Health Care Directive.  If you have already selected a health care agent, bring him or her with you to the class.  To get started now, visit kp.org/lifecareplan.
