Wellness coaches are specially trained to help you make lifestyle changes to reach your health goals. Talk with a wellness coach over the phone to build your motivation and create a customized plan to make lifestyle changes.
Life Care Planning
Life Care Planning is Kaiser Permanente’s name for a process also known as advance care planning. We believe that all adults should plan in advance.
Although it isn’t always easy, it’s important to have conversations about what you value most in life and how you would want to be treated in specific health or medical situations.
It is also important to document your wishes. Putting your plan in writing helps those caring for you to accurately remember your wishes so you get the care that you want. One way you can document your wishes is by completing an Advance Health Care Directive (AHCD).

Kaiser Permanente Santa Clara earns top teaching hospital, safety scores
Kaiser Permanente Santa Clara Medical Center has received the top score of “A” by the Leapfrog Group in its annual safety report, which examined and graded more than 2,600 hospitals throughout the United States. At the same time, Leapfrog named Kaiser Permanente Santa Clara one of the top teaching hospitals in the US, one of only 9 in California and one of only 53 in the United States.

Express Check-In is now available at Kaiser Permanente Santa Clara Medical Center and our Medical Offices at Campbell, Milpitas, Mountain View and Skyport/SanJose.
Pre-register for many eligible appointments and pay for your visit wherever you are — using your computer, tablet, or smart phone before arriving for your appointment.
When you arrive at the clinic, enter your medical record number (MRN) and date-of-birth (DOB) or scan your QR code at an Express Check-in station to notify us that you have arrived.
