Memory Orientation Class

If after viewing the videotaped Memory Orientation class, you are interested in receiving an individual memory evaluation at the Martinez Memory Center, please do the following:

• Ask your Primary Care Physician to refer you to the Memory Center
• Complete the Required Pre-Visit Questionnaire (below)
• Fax the completed Required Pre-Visit Questionnaire to 925-313-4588 or
• Email to: memorycenter@kp.org

Once the Martinez Memory Center receives a referral from your Primary Care Physician, someone from the Memory Center will contact you to schedule an in-person, individual, Memory Evaluation appointment.
