Clinical Case Management Services

Clinical Case Management Services provide ongoing care for patients who need more intensive individual, family treatment and case management, due to the severity and chronicity of their conditions. The family treatment is provided either with the patient, or through the Family Education and Support Group.

The patients followed by this team are typically diagnosed with Psychotic Spectrum Disorders, and Bipolar 1 Disorder.

Individuals referred to the CCM Team should meet criteria for medical necessity, as evidenced by level of severity, e.g. experiencing on-going psychosis secondary to schizophrenia or other psychotic disorder, mania, and serious symptoms impacting current level of functioning in the areas of socialization, occupation, and education.

Many of the CCM appointments are task oriented, e.g. linking patients to community resources, arranging collateral contacts with family members or care providers, coordination of clinic treatment modalities including referrals to classes, groups, the CCM group, the Young Adult CCM Group, the Life Skills Group, and medication appointments.

The CCM clinicians are also available to provide consultation about community resources to Adult Service staff.
