Browse our doctor profiles to see education, credentials, specialties, profile photos, and everything else you need to know to select a doctor who’s right for you. And if you change your mind, you can switch doctors at any time, for any reason.

How to Make an Appointment
Schedule an appointment online once you’ve registered and chosen a personal physician. If for any reason your personal physician isn’t available at your preferred time, you can see another doctor in our network. You can call our Appointment and Advice line at the number below at any time to make an appointment or seek medical advice.

Transfer Your Medical Records and Prescriptions
Prescriptions:
Bring your doctor up-to-date on your prescriptions and make it easy to get the medications you need from a Kaiser Permanente Pharmacy. You can work with one of our pharmacists by calling 1-877-795-2473.
Medical Records:
We suggest that you consult with your Kaiser Permanente Physician prior to requesting records from your previous health care provider.

Pedi Immunization Records:
Please send or bring in a copy of your child’s immunization records, so we know your child is protected with all the needed vaccines. We appreciate you providing these records as soon as possible. For your convenience, you can email us a copy of your child’s immunization records through kp.org and have them added to their medical records.

Interpreter and ADA-specific services are available in your preferred language at all times and at no cost to you. When you call us or come in for an appointment, we’ll make every effort to talk to you in the language you want to use.
Most of our facilities have bilingual staff that are trained to interpret and explain medical terms and procedures. Many of our physicians also speak more than one language. If one of our qualified staff does not speak your language, we can arrange for an in-person interpreter, or provide interpreter services in more than 140 languages by telephone.
We can also provide you, your family and friends any special assistance to access our facilities and services, including appropriate accommodations or alternative reading format of member information. Just let us know how we can help.

Manage your health information
If you need copies of your health information for your own personal use or to forward to a health care provider or organization, Kaiser Permanente’s Release of Medical Information Departments are here to help you.
Your health – anytime, anywhere
Some of your medical records are already available to you online – anytime, anywhere – with My Health Manager on kp.org. As a registered user, you can review, download, and save to your computer or print the following information:
- Immunization record
- Downloadable Clinical Health Summary
- Hospital Summary
- Past office visits
- Prescription medications
- Most lab test results
- Your current health summary, including any allergies and ongoing health conditions

Requesting your records
For your convenience, you may request your records using phone, email, or by visiting any of our locations. Our offices are generally open from 8:30 a.m. to 5:00 p.m. Please follow the link for the facility closest to you.
Release of your records
Fast and Convenient: You can obtain a copy of your recent medical records via email. We can provide copies of your most recent 2 years’ worth of records via email for no fee (free) and they can generally be emailed the same day. A minimal charge applies for records greater than 2 years that are emailed or provided on CD. If you need records on paper, a per page fee will be charged.
Third party authorization
To protect your privacy, your records will not be released to another party without your signed authorization. You will need to submit a disclosure authorization form for us to complete the request. For your convenience we have these forms located on kp.org under Locate our services, then Forms and Publications.

Disability and family leave
We assist your physician in completing disability and Family Medical Leave certifications, too. We will need a physician’s note for time off to assist in completing your paperwork. You may also need to have a visit or discussion with your physician, if you haven’t had one recently, prior to submitting your paperwork. Click the link to complete the Online application for State Disability Insurance. Please retain a copy of your claim receipt and provide it to the Release of Medical Information staff when requesting State Disability Insurance physician certifications.
Forms and correspondence
Working with your personal physician’s office, we can help you complete various forms regarding your health care, including:
- Forms to verify illness or health condition
- School sports clearance
- Department of Motor Vehicles
- Jury duty
- PG&E
- Residential Board and Care forms
Completed forms are returned to you, rather than to a third party, so you can review the information and make a copy for your records first.
