Care Philosophy:
Your physician or nurse practitioner may have referred you to our program because you have been having difficulty managing your diabetes as measured by a blood test called HgA1C (greater than or equal to 8.0%).
We specialize in helping you manage your diabetes so you can lead a healthy and active life. We will work with you to develop a personalized care program to help you learn the skills you need to get your diabetes in control and to keep it there. This program will work best if you take an active part in managing your diabetes. We highly recommend that you attend basic diabetes education classes before joining the program.
In order to ensure the best quality of care and continuity of care we have a diabetes team consisting of two physicians, specially trained registered nurses, a registered dietitian, a clinical pharmacist, a behavioral psychologist, two program assistants, and two dedicated volunteers.
