Here is information to help make your transition into our health care system as smooth as possible. Our members can access services at any of our locations as soon as your coverage becomes effective. If you aren’t sure of your exact effective date, please ask your employer.

Your Kaiser Permanente identification card and informational material will be sent when your coverage becomes effective. However, since you may need health services as soon as your coverage is effective, the following information can help you get started.

If you would like a tour of the facility, call Member Outreach at 650-299-4291 to make an appointment. And we recommend that you familiarize yourself with our campus map (English) (Español) (note the free parking garage) and our shuttle service.

Printed on your Kaiser Permanente ID card will be your Medical Record Number. This card will be mailed to you. The MR number will be the point of reference for all care provided to you.
If your health coverage is effective and you need care before your card arrives, you can request your Medical Record Number. Call Member Services.

For advice or to make an appointment, call the number listed below. It’s helpful if you bring a copy of your enrollment form to your appointment if you have not received your Kaiser Permanente ID Card.
If you have received your Medical Record Number, selected a physician, and registered for kp.org, you can make an appointment online.

As a member, you may transfer a prescription from a non-Kaiser Permanente pharmacy to any of our pharmacies. Be ready to tell the Kaiser Permanente pharmacist your prescription number and the name and phone number of the outside pharmacy. Our Kaiser Permanente pharmacy team will handle the rest.
This transfer can take up to 48 hours. To use this service, simply call the Kaiser Permanente pharmacy call center toll-free at 877-RX-KAISER (877-795-2473).

When you visit your personal physician for the first time, talk to him or her about your health history and goals. Your doctor can determine what medical information may need to be transferred from your previous health care provider.
New members should receive a Medical Record Number from Kaiser Permanente before completing an Authorization for Disclosure Form and transferring medical records. Please make sure you have your Kaiser Permanente Medical Record Number on the form before sending materials.
Complete an Authorization for Disclosure Form (Spanish version) (Chinese version), and take it to your non-Kaiser Permanente physician. We recommend that you keep a copy of your important medical information at home.

complementary health services like massage therapy, acupuncture, and
chiropractic services as well as preferred rates on select fitness club

My Doctor Online provides you with a convenient way to stay connected with your doctor. Several of the new features of your physician’s home page include:
- Background information about your doctor
- Information about diseases and conditions
- Information about how to prepare for your office visit or procedure
- Commonly used forms
- Answers to many frequently asked questions
- A wide variety of health tools and classes including videos, podcasts, and interactive programs on various health topics such as diabetes, cholesterol, hypertension and more.
Help in your language
When you call us or come in for your appointment, we will try to speak to you in the language you are most comfortable with.
Our call centers have interpreters who speak Spanish, Cantonese, Mandarin and other Chinese dialects. Most of our offices and hospital have people who speak more than one language, as well as American Sign Language interpreters who are specially trained to interpret and explain medical terms and procedures. Many of our doctors also speak more than one language. If no one at the facility you visit speaks your language, we have interpreters for more than 140 languages available by phone.
You can call Member Outreach at 650-299-4291 to update your medical record requesting an interpreter for the visit.
