Personal Health Coach Program
The Personal Health Coach Program is offered at Kaiser Permanente San Francisco Medical Center’s Health Education Department. We provide one on one coaching to help optimize your health and wellness goals through nutrition and fitness coaching by a Registered Dietitian and a Physical Therapist / Certified Personal Trainer.

Wellness Coaching by Telephone
Call 866-251-4514 or make an appointment online to talk with a Wellness Coach by phone.  Available in English and Spanish for members 18 and older.  No fee.

Meet with a Clinical Health Educator – Adults 18 and older
Make an in-person, video, or phone appointment with a Clinical Health Educator. Our Clinical Health Educators are available to provide individual education and counseling for a range of health concerns. Education topics include weight management, general nutrition, exercise, smoking cessation, stress management, cholesterol, hypertension, and prediabetes care.

Available in English, Spanish, and Cantonese for members 18 and older. No fee. To schedule, call 415-833-3450.

Meet with a Clinical Health Educator – Children and Families
Make an in-person or phone appointment with a Pediatric Clinical Health Educator. Our Pediatric Clinical Health Educators are available to provide education and counseling on a range of health concerns from children of all ages, teens, and families. Education topics include pediatric weight management (healthy eating, physical activity, screen time), general healthy eating, pre-diabetes, sleep concerns, picky eating, and toilet training.

Available in English and Spanish for members 17 and younger and their families.
