Member Photo: Get your photo taken for your secure electronic medical record!
As part of a voluntary program you can have your photo taken during today’s visit to our Medical Offices. The photo will be permanently included in your secure electronic medical record.
This will allow your doctor and other care providers to become more familiar with you before and during your visit.
Get your photo taken Monday-Friday at:
The Healthy Living Center, 9:00 a.m. – 5:00 p.m.
The Technology Learning Center, 10:00 a.m. – 4:00 p.m.
Please note: For the protection of your identity and the security of your medical record, we still need to see your Kaiser card and a photo ID each time you register. We also recommend that children under 10 years of age not be photographed, due to frequent changes in their appearance as they grow up.
