Getting Your Medical Record Number

Your Kaiser Permanente Medical Record Number is printed on your Kaiser Permanente ID card. The card will be mailed to you.

If your health coverage is effective and you need care before your card arrives by mail, however, you may request your Medical Record Number by calling Member Services
