Here is information to help make your transition into our health care system as smooth as possible. Our members can access services at any of our locations as soon as your coverage becomes effective. If you aren’t sure of your exact effective date, please ask your employer.

As a member, you can access services at any of our locations as soon as your health care coverage starts. If you aren’t sure of your start date, call Member Services.
Your Kaiser Permanente identification card and informational material will be mailed to you when your coverage starts. The following information can help you get started.

For advice or to make an appointment, call the number listed below. Please bring a copy of your enrollment form to your appointment if you have not received your Kaiser Permanente identification Card, which contains your medical record number.
If your health coverage has started and you need care before your card arrives, you can request your Medical Record Number by calling Member Services.

As a member, you may transfer a prescription from a non-Kaiser Permanente pharmacy to any of our pharmacies. Our pharmacy will need your prescription number and the name and phone number of the pharmacy that filled your prescription to complete the transfer.
This transfer can take up to 48 hours. To use this service, call the Kaiser Permanente pharmacy call center toll-free at 877-RX-KAISER (877-795-2473).

When you visit your personal physician for the first time, talk to him or her about your health history and goals. Your doctor can determine what medical information may need to be transferred from your previous health care provider.
New members should receive a Medical Record Number from Kaiser Permanente before completing an Authorization for Disclosure Form and transferring medical records. Please make sure you have your Kaiser Permanente Medical Record Number on the form before sending materials.
Complete an Authorization for Disclosure Form (Spanish version) (Chinese version), and take it to your non-Kaiser Permanente physician. We recommend that you keep a copy of your important medical information at home.

Kaiser Permanente recommends that all members fill out an Advance Directives form to ensure that your desires regarding your health care are met. You can download and print the forms, or pick up hard copies.

Through our comprehensive Web site, kp.org, Kaiser Permanente offers our members a simple and safe way to manage health care services, communicate with their personal physicians, and much more. Using kp.org, you can:
- E-mail your doctor.* After you have selected your personal physician, you can
e-mail your doctor routine health questions from the comfort and convenience of
your computer. - E-mail your child’s or other family member’s doctor, and access other secure
features on their behalf, using the “Act for a family member” feature.* - Order prescription refills for yourself and your family members and have most
mailed to your home for no additional charge.* - Schedule routine appointments for yourself and your family members.*
- View most lab test results, past office visit information, and recent immunization
history.* - Get information about your health plan and benefits, download forms, and more.*
- Use personalized online tools to help you develop a plan to manage your weight,
reduce stress, improve nutrition, stop smoking, and assess your lifestyle habits. - Enjoy a 25 percent reduction in rates from select providers of
complementary health services like massage therapy, acupuncture, and

Our South San Francisco Medical Center provides complimentary interpreter services for our members 24 hours a day, seven day a week. You can request an interpreter when you schedule your next appointment or you can call Interpreter Services directly:

When possible, we will arrange for interpreters in person, however a phone interpretation is always available—even on short notice. You can ask us to arrange a telephone call with you, your doctor, and an interpreter.
