Bridge Program

This program offers the specialized care patients need to recover independent living skills, while letting them return each day to the comfort and familiarity of home.

• A board-certified physiatrist (a physician specializing in physical medicine and rehabilitation) supervises the patient’s care.
• The care team may include a rehabilitation nurse, social worker, physical therapist, occupational therapist, speech pathologist, and neuropsychologist—to meet all the patient’s rehabilitation needs.
• The care team, the patient, you, and your family work together to create a treatment plan that meets the patient’s needs and goals.
