Multi-Purpose Senior Service Program (MSSP)The California Health Collaborative MSSP is a non-profit, community-based program that provides case management services to the frail and low-income seniors in Sacramento, Placer and Yolo Counties. MSSP strives to keep seniors safely at home for as long as possible. The program is staffed by Registered Nurse Care Managers, Social Worker Care Managers, and Case Aides who provide monthly monitoring and assistance. Who is eligible for MSSP Services?

Please feel free to contact Thomas with concerns or questions. Oncology Support GroupThe Oncology support group is available for anyone living with cancer in addition to their family and friends. Facing a diagnosis of cancer is often overwhelming. A support group can help. This on-going support group provides a confidential place to discuss the wide range of emotions experienced following a cancer diagnosis. the supportive group atmosphere allows members to discuss their feelings, share information and talk about coping with cancer. Group is conducted the 2nd and 4th Tuesday of the month, beginning 9/11/2018, from 2:00 PM – 3 PM at the Downtown Commons building, 501 J Street, Sacramento, Ca 95814, conference room 160. |
