Asthma/COPD
The Asthma/COPD Care Management Program at the Kaiser Permanente Santa Clara Medical Center is a special program designed to provide the highest quality of care to our members with asthma and/or COPD.
Our specially trained Pharmacists and Respiratory Care Managers partner with our doctors to help members manage their asthma and COPD in the best way possible. Our goal is to help members succeed in managing their asthma or COPD, so they can lead healthy active lives.
What to expect from the program
- Development of personalized self management plan for asthma and COPD control and maintenance
- Learn ways to reduce exposure to allergens, irritants, and infections that may trigger asthma or COPD
- Monitoring of peak flow readings and asthma symptoms
- How to make the best use of asthma/COPD medications
- How to manage asthma/COPD flare-ups
- Learn how to improve asthma/COPD control in ways that fit your lifestyle
- Gain self care skills necessary to live a healthy and active life
SUPPORT GROUPS AND CLINICS
Drop-in Support group
Asthma/COPD Drop-In Support Group is available on a weekly basis at our Homestead facility
in department 196 (Room A3/A4).
- The 1st and 3rd Tuesday of the month the group starts at 3 p.m.
- The 2nd Tuesday of the month the group starts at 3:30 p.m.
- The 4th Tuesday of the month the group starts at 3:15 p.m.
* No appointment is required and there is no co-pay for this group visit.
- During the 1 hour support group, you will have the opportunity to visit with a physician
and a care manger, ask questions regarding your health, and learn how others deal with
their chronic respiratory problems.

Sleep Clinic Drop-In (for patients with NCPAP machine)
- Every Tuesday at 10:30 to 11:30 p.m. and every Thursday at 2 to 3 p.m. in Department 266
at our Homestead facility.
Bronchiectasis Support
- This support group takes place the 4th Tuesday of every month from 2 to 3 p.m. in
Department 196, rooms B1/B2 at our Homestead facility.
