Fracture Prevention
The Fracture Prevention Service at the Kaiser Permanente Santa Clara Medical Center is a special service designed to provide the highest quality of care to our members who have had a recent bone fracture. Our specially trained pharmacists partner with our doctors to help members prevent future fractures and falls in the best way possible. Our goal is to treat and educate patients to support bone health through nutrition and medication to reduce risks of another fracture so they can lead healthy, active lives.
What can members expect from the program?
- Personal care manager communication via telephone and/or secure emails
- Testing blood and bone density scans to evaluate bone health
- Development of personalized self-management plan for fracture prevention
- Information on preventing future fractures
- Medications for fracture prevention
- No fees or co-pays for participation in program
