Life Care Planning (Advance Directive)
Life care planning begins with choosing someone to speak on your behalf if you were ever unable to communicate for yourself. This class can assist you with selecting your
health care agent and exploring your values and goals for care. You will also have an opportunity to complete an Advance Health Care Directive. If you have already selected a health care agent, bring him or her with you to the class.

Health Counseling
Our health educators are available to provide individual education and counseling for a range of health concerns for adults and children. Education topics include weight management, general nutrition, exercise, smoking cessation, insomnia, parenting, cholesterol, hypertension, and diabetes care. In person, video, and phone appointments are available. Available in Spanish.
