Perioperative Medicine (POM) Clinic

It is important to keep this appointment in order to prevent issues that may cause a delay or cancellation of your surgery. The date and time of your appointment will have been sent to you in an e-mail or given to you by one of our surgery schedulers. If you are scheduled to receive a phone call from the perioperative medicine physician or nurse practitioner, please be available 1 hour before and 1 hour after the appointment time. Be prepared to discuss the following information with them:

• The medications you are currently taking, including the times you take them and the dosage. Please include any over-the-counter medications, vitamins, and herbal supplements.
• Any allergies to medication and/or food.
• Any previous surgeries and the dates of those surgeries.
• Your medical history (i.e., history of diabetes, high blood pressure, heart disease, etc.).
• Your family history. (i.e., family member who has history of cancer, diabetes, heart disease, etc.)

All perioperative medicine patients are asked to:

• have a kp.org password, so they can send and receive secure e-mail messages from their clinicians, including their POM and anesthesia clinicians. Members with a kp.org password can also check test results, refill prescriptions, make primary care appointments and view preventive reminders and past visit information – all from their clinicians’ home pages at kp.org/mydoctor. • have an advance health care directive, a legal document which allows you to tell others what kind of healthcare you want to receive in an emergency or when you are unable to make decisions about your care. Please review frequently asked questions and answers about advance care planning.

Please bring your current medications or have an accurate list of your medications available.

During your appointment, we will review your medical record and preoperative tests, and  update your current medications list. Additionally, lab tests, EKG’s, or other studies may be ordered to help optimize your health prior to surgery. At the conclusion of this visit, you will be provided with important information and specific instructions to help you prepare for your surgery.

Here are some Preoperative Instructions, which will be discussed in the POM visit.

If you have an active kp.org password, you can expect to receive an e-mail from your anesthesia clinician a few days prior to surgery. If not, you should receive a letter by mail from your anesthesia clinician. On the day of surgery, you will meet your anesthesia clinician and can discuss any questions you may have.  In the meantime, you may find the information below helpful:

Pre-admission expedites the admission process on the day of your surgery and ensures all information (e.g. emergency contacts, benefit information, financial responsibility, language preference, etc.) is accurate and current.

If you are scheduled for a procedure/surgery at Kaiser Permanente San Jose, the Eligibility and Financial Services (EFS) office will call you and complete your pre-admission procedure over the phone. If you have not received a call from EFS within 2 business days prior to your procedure, a KP San Jose Admitting Representative will call you instead. Pre-admission may involve a collection of co-payment and deductible fees.

Please:

• be prepared to provide your personal information (e.g., social security number, emergency contact, address, phone numbers, insured person’s information) to an admitting representative.
• be prepared to pay your cost share by credit card over the phone.
• note that forms will need to be signed on the day of your procedure/surgery.
• note that if an admitting representative is unavailable to pre-admit you when you call, we will take your number and call you back.

Next, we would like you to preview each step of your procedure online with the animated, interactive Prepare for Your Procedure (EMMI) program.  This short, easy-to-understand video will provide you with a clear sense of what to expect before, during and after your procedure. Patients who watch these find them very helpful and feel reassured about their experience having surgery.  The video can be viewed at home, at a Kaiser Permanente Health Education Center, or anywhere with internet access. Many can also be watched on an Apple iPad. Please watch the program as many times as you would like and share it with family and friends.

PLEASE NOTE: NOT EVERY PROCEDURE WILL BE LISTED.  IF YOU DO NOT FIND THE PROCEDURE YOU ARE SCHEDULED FOR, YOU MAY STILL FIND IT HELPFUL TO VIEW THE ANESTHESIA EMMI PROGRAM.

A Kaiser Permanente surgery scheduler will call you 3 business days before your surgery to confirm your surgery arrival time and location. If you do not receive this call by 12 noon on the business day before surgery, please contact the surgery scheduler associated with your surgeon.
