Name and Gender Marker Change
In California, a person can change their legal name and gender marker or a minor’s name and gender by getting a court order. It is not necessary to provide any medical documentation in order to obtain a court-ordered change. Gender marker can be changed to male, female, or nonbinary.
California Courts Gender Change
With a court order a person can change their legal name and gender marker on government-issued identification documents:
- Driver’s license at the DMV
- Social security number with the Social Security Administration
- California birth certificate with the Office of Vital Records (out of state requirements may differ)
- US Passport – the State Department requires a letter from your medical physician on office letterhead that states that you have had “appropriate clinical treatment for gender transition.”

Visit the KP Name and Gender Change site to learn more about changing legal names and gender markers with Kaiser Permanente medical records. Please note, if medical coverage is provided through an employer-based health plan, name and gender marker must be first changed with the employer before contacting Kaiser Permanente Member Services.
