National Health Care Decision Day

Kaiser Permanente West Los Angeles will be offering virtual Life Care Planning workshops for Kaiser Permanente members who are active on kp.org. We will also offer telephone appointments with a Life Care Planning Facilitator to walk the member through life care planning steps and how to complete the Advanced Directive.

Interested participants can call the Center for Healthy Living office at Baldwin Hills Crenshaw to request an appointment for either a virtual or a one-on-one consultation: (323) 421-2710.

Due to telephonic/virtual modalities, we are not able to accept non-Kaiser Permanente members into services at this point.
