Stay up-to-date with preventive care in Colorado
When you and your loved ones visit our medical offices for checkups and other visits, we’ll recommend any screenings and immunizations you might need to stay healthy. When you’re not visiting, you can view your personal action plan on kp.org to help you stay on track.
Your personal action plan is an online tool that gives you personalized information about any preventive services you need, based on your age, gender, and health history. Find out where you stand, what actions you should take, and how to complete the screenings or lab orders you need.
To view your personal action plan, or the plan of an adult family member whom you’ve added to your family list, go to kp.org/actionplan and sign on. You’ll be taken to “My health manager.” Click on “My medical record,” then choose “Personal action plan” from the menu on the left.
