Take charge of your health in Colorado
Have you seen your Personal Action Plan on kp.org? This new online tool for our Colorado members gives you customized information about the screenings and preventive services you are personally due for based on your age, gender, and health history.
The plan makes it easy for you to see where you stand and what actions you need to take now and in the future. It also tells you how to take the next step by making a routine appointment, completing a lab order, getting an immunization, and more.
Based on what’s relevant to you, the plan shows your status on cancer screenings, preventive care, heart health, ongoing health conditions and medications, immunizations, and general guidelines.* The Personal Action Plan will become more advanced over time as more topics and information are added.
