The Outpatient Palliative Care Clinic works together with a patient’s doctors and care teams to provide an extra layer of support for people living with a serious illness.

• Our team includes a doctor, a nurse, and a social worker.
• Our goal is to help you and your loved ones live as well as possible every day.
• We can work with your healthcare providers and family to make communication smoother, and plan for the future.
• We can also help you manage your physical and emotional symptoms that come with having an advanced illness.
