Community Based Medicine

We are a group of geriatric physicians and geriatric nurse practitioners who provide home-based primary care for homebound frail Health Plan members. We also provide care for members in Skilled Nursing Facilities, whether there for short-term skilled nursing needs or rehabilitation, or as long-term residents. Nurse case managers, social workers and a receptionist help us provide these services.

Since we see patients only out in the community, we do not have an office for purposes of patient visits. We see patient by referral only.
