Choose your doctor at 888-956-1616
A personal physician is someone you get to know and trust. Start building that relationship with a toll-free phone call to select the right doctor for you and schedule your first appointment. Available 7am to 7pm, Monday through Friday. At your visit, your doctor may suggest some important preventive health screenings, like blood pressure, cholesterol, diabetes, and tests for certain cancers. Be sure to discuss any special health needs and if certain conditions run in your family. This data will be entered into your electronic health record, which will help us personalize your care.
Register for secure access to kp.org
With your Kaiser Permanente identification card handy, go to kp.org/registernow to set up your user ID and password. Once you’ve registered, you’ll have free, around-the-clock access to the time-saving features of My Health Manager. These convenient tools can help you manage your care at Kaiser Permanente facilities. E-mail your doctor’s office, schedule routine appointments, order prescription refills, view most lab test results, and more–day or night.
Take a total health assessment
After you’ve registered online, visit kp.org/healthylifestyles to take a total health assessment with HealthMedia® Succeed™. This program will analyze your lifestyle choices and create a customized health improvement plan to fit your needs. You can even attach the results to your electronic health record so your doctor can work with you to help you reach your goals.
