The steps below will help you understand how to apply for Medicaid in Oregon and get care through Kaiser Permanente for yourself or your child.

You may qualify for free or low-cost coverage through Medicaid based on your income, family size, and age.

What is free or low-cost health coverage?

Fill out your information below to find out if you may qualify for Medicaid. If you do, you may be eligible to get your Medicaid health care through Kaiser Permanente. If you don’t qualify for Medicaid, you may qualify for federal financial assistance to help you afford your health care.

NOTE: The information provided is for reference only. Final eligibility is determined by the Oregon Health Plan for Medicaid and one.oregon.gov for federal financial assistance.

All the family members who live with you. If you file taxes, this means everyone on your tax return.

By Mail Print an Oregon Health Plan application online from one.oregon.gov. Send completed and signed applications to the address listed on the application.

By Phone For information about applying and enrolling in the Oregon Health Plan (Medicaid), please call OHP Customer Service at 1-800-699-90751-800-699-9075 (TTY 711711).

In addition to your completed Medicaid application, you may be asked to provide other documents, such as:

Identification
Proof of U.S. Citizenship or Immigration Status
Proof of residency (you must live in the state in which you apply)
Income status

4. Receive approval for Medicaid.

After you’ve been approved for Medicaid, here’s what you need to do to select Medicaid with Kaiser Permanente:

5. Be assigned to a Coordinated Care Organization (CCO) and choose Kaiser Permanente as your healthcare provider.

The Oregon Health Plan will assign you to a CCO.

In order to have Kaiser Permanente as your healthcare provider you must be assigned to Health Share of Oregon (HSO), Trillium Community Health Plan, or PacificSource Community Solutions as your Coordinated Care Organization (CCO). If you are assigned to another CCO, you may be able to change this in the first 30 to 90 days by calling the Oregon Health Plan at 1-800-699-90751-800-699-9075 (TTY 711711).

Kaiser Permanente may or may not be accepting new members.

If Kaiser Permanente is not accepting new members, an exception may be available if you:

Have an immediate family member who is a Kaiser Permanente member, OR
Are or were a Kaiser Permanente member in the last 12 months

What is an immediate family member?

Immediate family member
An immediate family member is a family member who lives in the home of a current Kaiser Permanente member and is a:

Spouse (including Domestic Partners)
An unmarried dependent child under age 21
A disabled dependent over the age of 21
Married/Unmarried/Stepparents of children under age 21
Foster child or stepchild
Legal guardian
A grandparent, parent, guardian or other relative who applied for Medicaid on behalf of a current Kaiser Permanente member that is under 21.
