How to Qualify and Apply for Medicaid in Georgia

By Mail  Obtain a Medicaid application from your local DFCS office. Send completed and signed applications to:

In addition to your completed Medicaid application, you may be asked to provide additional documents such as:

• Identification
• Proof of U.S. Citizenship or Immigration Status
• Proof of residency (you must live in the state in which you apply)
• Household income

It could take the state 45 days to see if your child/children are eligible for Medicaid coverage. To check on the status of your child/children’s application, visit gateway.ga.gov or call 1-877-423-47461-877-423-4746.

Shortly after you are approved for Medicaid or PeachCare for Kids® you will be enrolled into a health plan in Georgia Families. Georgia Families will mail you a welcome letter with information about your health plan. If you do not want the health plan that has been chosen for you, you can change to another health plan. You will have 90 days from the start date of your health plan to change to a new health plan.

In order to choose a Kaiser Permanente doctor:

1. 1. First, find the Kaiser Permanente facility that is most convenient for you. Make a note of the facility’s address.
   2.  Then, contact the Care Management Organization (CMO) you are assigned to and tell them the address of your preferred facility. Your CMO can see which providers are accepting new patients.

Kaiser Permanente may or may not be open to new Medicaid patients depending on our contract with our plan partners, WellCare, Amerigroup, Peach State, and CareSource.

Your child/children’s coverage will begin on the first day of the month after your child/children are enrolled.

Medicaid recipients will receive one ID card from your Care Management Organization with Kaiser Permanente information on it.

PeachCare for Kids recipients will receive two ID cards – one from your Care Management Organization, and one from PeachCare for Kids.

Please continue to bring your child/children’s Georgia Medicaid card to their appointments even after you receive their CMO ID card(s).
