Geriatric Medicine Staff

The Geriatric Medicine team offers consultative geriatric evaluation and management (GEM) for older adults, their families/care partners, and their primary care physician. The evaluation provides a comprehensive assessment of an older adult’s health conditions, including dementia, frailty, and multiple medication use, in the context of social and family needs. We provide a complete plan for managing the person’s conditions and care, providing education and patient-specific information about health problems. The team works together to develop a personal evidence-based treatment plan that addresses not only the needs of patients but also their family and caregivers, including level of care consultation.

Our goal is to provide our frail geriatric patients with education, services, resources, support, and guidance every step of the way, to help improve the quality of their lives, as well as the lives of their caregivers.

How do I contact the Geriatric Medicine Department?

Please complete the pre-visit questionnaire prior to your appointment. All information obtained through the geriatric evaluation and management clinic will be shared with the referring physician.

The Geriatric Medicine team works in collaboration with your primary care physician, who remains in charge of your care.
