DMV Report of Vision Examination
If you have failed the DMV eye test and they require you to have an eye doctor evaluation, print the DMV vision examination report form and schedule an appointment with an ophthalmologist or optometrist to do the exam. They’ll fill out the report for you to bring back to the DMV.
