Your Doctor, Kaiser Permanente Card and Online Account

Choosing Your Doctor
When you turn 18 and/or graduate from high school, you can choose your own primary care doctor from (FMS). Girls can also choose a doctor from the OB GYN department. You can read more about each of our and OB GYN doctors to help you choose one (or two) who will care for you on an ongoing basis.
To get the best care, you’ll want to find a doctor you can comfortably talk to about any problems or questions. Once you find one you really like, try to stay with them so they can get to know you and your medical history. You can change your primary care doctor(s) whenever you want and as many times as you want, no questions asked.
Not all of our physicians are able to take new patients all the time. Some of them will already have the maximum number of patients at the time you are picking a doctor. Their panels are always changing so check back later to see if the doctor you want has any new openings. You can look up which physician has openings and officially select your primary care doctor.
What to Bring to an Appointment
When you come to Kaiser Permanente Santa Rosa for an appointment in any department, you should bring a photo ID, your Kaiser Permanente card (or your Kaiser Permanente number) and money for your co-pay and any prescriptions you might need to pick up.
How To Get Your Own Kaiser Permanente Card
You can request a copy of your card when you’re in Pediatrics.
You can call Member Services at 1-800-464-4000, Monday through Friday, 7 a.m. to 7 p.m., Saturday through Sunday, 3 a.m. to 3 p.m. If you don’t know your Kaiser Permanente member number, you’ll need to call during business hours, 9 a.m. to 5 p.m.
No matter when you call, you’re going to be connected to an automated phone system. Here’s how to get through it as quickly as possible:
- If calling during business hours, choose “Current Member” (option #1), then “Other Inquiry” (option #4), then “Order ID Card” (option #2). You’ll need to know your birthday and the zip code of the person who pays for your insurance.
- If calling after business hours, choose “Current Member” (option #1), then “Order ID Card or Other Forms and Publications” (option #1). Hit #1 to confirm. You’ll need to know your Kaiser Permanente number and the zip code of the person who pays for your insurance.
You can request a card online.
NOTE: your card will be sent to the home of the person who pays for your insurance (the person who’s name your account is listed under). You can’t have your card sent to a different location or pick it up at a Kaiser Permanente facility.
How To Get a Username and Password for kp.org
At age 13, you can sign up for your own kp.org website account that enables you to email your doctor, schedule appointments online, refill prescriptions and have them mailed to you, and view your medical record. Be sure to make a note of your username and password so you don’t forget it and don’t share it with anyone.
